Registered Nurse - Clinical Liaison
$20K Sign-on Bonus
4 (10s) every other weekend


Job Summary:


Working in an assigned territory, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families, and working with healthcare professionals (case managers, discharge planners) to ensure optimal transition from hospital or other facilities to home. Responsible for overall referral growth in assigned territory and facilities. Supports admissions staff by admitting patients inside and outside assigned territory as requested.


Job Duties



  • Makes initial contact and physical, social and psychological and financial assessment of patients to determine appropriateness of hospice services.

  • Self identifies options for payment, which are not necessarily identified by patient, including assistance with financial application.

  • Determines needs and coping skills of caregiver in caring for patient.

  • Interprets policies and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.

  • Demonstrates the ability to tailor plan of care based on unique requirements of the patients age and developmental needs. Coordinates care plan, suggesting appropriate services to family and staff.

  • Initiates referral-to-admission process for each patient referred by facilitating required data, leveraging available resources to ensure data is complete and accurate.

  • As needed, provides accurate and thorough documentation either via phone, fax, or computer to office for additional data entry.

  • Collects appropriate signed forms.

  • Provides accurate and thorough documentation in medical record.

  • Ensures documentation is legible and comprehensive.

  • Maintains confidentially and follows HIPAA regulations.

  • Handles patient problems as related to unmet expectations for hospice.

  • Effectively communicates with physicians who are non-supporters of organizations services.

  • Communicates with families regarding changes in care plan activities.

  • Conducts individual and group presentations on products and services in an informative, accurate, and professional manner. Responds to questions about products and services.

  • Successfully builds relationships with case managers, social workers, and other care providers.

  • Acts as liaison between physicians, skilled nursing facilities, hospitals and rehabilitation providers communicating service enhancements, new products, new programs and service delivery.

  • Facilitates communication between Business Development and Hospital, Skilled Nursing Facility representatives, and other referral sources. Relates opportunities for recovery activities.

  • Keeps agency informed of facility changes regarding bed capacity, staff changes, new facility programs, and/or procedures.

  • Understands that teamwork is a crucial part of our business, and is able to work well with colleagues, no matter the circumstances. Using individual skills, cooperates with others, and accepts and provides constructive feedback despite any personal conflicts between individuals involved.

  • Utilizes current agency and/or department specific software to complete assignments.

  • Completes admission initiation visits and start of care visits as needed.

  • All other duties as assigned

Job Requirements:





Computer Skills: Strong computer skills including database and EMR software and Microsoft programs.


Supervisory Responsibilities: This position has no direct reports.


Education/Experience: Graduate from an accredited school of nursing. BSN preferred and two years of experience working in an acute care setting; one year of home care and/or hospice experience preferred. Working knowledge of community resources. Ability to meet, collaborate and work efficiently with others in the community. Knowledge of entitlement programs.


Language Skills: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to prepare educational presentations that conform to prescribed style and format. Ability to effectively present information to public groups, facilities, hospitals, and community.


Mathematical Skills: Ability to work with mathematical concepts such as add, subtract, fractions, percentages, and ratios.


Reasoning Ability: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret a variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.


Certificates, Licenses, Registrations: Current nursing license. Current driver’s license state of Florida. Possession of a registered and insured motor vehicle. CPR certification. CHPN certification preferred.
