As our Case Manager, you will facilitate and
process new patient referrals, utilizing advanced nursing skills in the
assessment and planning of clinical care for the international patient and
other identified group of patients.  You will review patients’ medical conditions with their home country physicians
to coordinate, clarify needs and expectations as well as to coordinate transfer
of care back to home country physicians after receiving care at UCLA.  You will also coordinate with physicians, staff, and
non-UCLA providers and facilities regarding patient care and provide medical updates to embassy sponsors and other
payors on a regular and as needed basis.

* RN required; BSN required, MSN preferred.
* Demonstrated experience in acute care hospital
or the equivalent of education and experience.
* Recent experience in case
management, utilization management, or discharge planning.
* Team leading or
management experience.
* Clinical experience sufficient to understand and
communicate medical diagnosis and courses of treatment to professional and
non-professional personnel.  
* Demonstrated
ability to understand and conduct a complex health intervention system.  
* Demonstrated knowledge of and experience with
EPIC/CareConnect preferred.  
* Demonstrated experience of PC applications
including Word, Outlook, Access, Excel required.
* Knowledge of and/or experience
with Salesforce CRM preferred.
*  Ability to work effectively and
collaboratively with interdisciplinary teams.
* Ability to work flexible hours to complete
tasks and meet deadlines.
* Strong written and verbal communication skills.
