Job Title: Diabetes Case Manager Nurse

Schedule: 40 hours/week, Monday – Friday (8:00am- 4:30pm)
Location: Valley Center, CA
Pay range: $82,815 to $89,005 annually DOE

SUMMARY
Through home visits and community events, the Diabetes Nurse Case Manger instructs individuals and families in health education and disease prevention. The Diabetes Nurse Case Manger also serves as the lead Diabetes Case Manager overseeing other diabetes case managers. The Diabetes Nurse Case Manger participates in all diabetes related activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Include the following. Other duties may be assigned.
1. Conducts home visits to appropriate enrolled IHC patients to determine patient needs and provides Public Health Nursing services. Instructs family in care and rehabilitation of patient and in the maintenance of health and prevention of disease for individuals, family members and the community.
2. Develops, plans and implements a coordinated patient - centered care plan for individuals and families including the establishment of priority health needs and evaluation of care. Instructs individuals, families and caregivers to recognize early signs of disease and deviations from normal health and how to initiate appropriate action.
3. Serves as the Diabetes Management (DM) Team Lead Case Manager. Manages the productivity and performance of other DM Team members as related to the SDPI grant objectives (Registered Dietitian, Physical Activity Specialist). Responsible for meeting grant objectives of the SDPI (Special Diabetes Program for Indians) grant.
4. Attends appropriate internal and external partnership meetings on behalf of Public Health Programs, as assigned.
5. Participates in community health screenings by performing screening tests as authorized and interprets findings to individuals and/or families. Assists participants in identifying goals and activities that will lead to improved health outcome.
6. As a Diabetes Team member, participates in all aspects of the Diabetes program and other programs. Activities include Continuous Quality Improvement (CQI), Diabetes Case Management, Intense Case Management, Interdepartmental Collaboratives, and other associated committees and staff meetings.
7. Submits all patient documentation according to IHC Policies and Procedures within the appropriate timeframe.
8. Interprets social and physical factors in the environment that affect the patient's care. Recognizes attitudes and cultural patterns related to health and researches culturally appropriate informational and educational materials, guides and brochures.
9. Participates in educational activities including Quarterly IHC staff and departmental meetings, mandatory IHC training and continuing education.
10. Assists in the design and implementation of field investigations and research projects as assigned.
11. Participates in Peer Review.
SUPERVISORY RESPONSIBILITIES
Manages one or more subordinate employees. Responsible for the direction, coordination, and evaluation of subordinates. Carries out supervisory responsibilities in accordance with the organizations' policies and applicable laws. Responsibilities include interviewing and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
TRAVEL & DRIVING REQUIREMENTS
Travel is not required for this position.
Driving is required for this position. Must be familiar and comfortable driving around the service area of the nine reservations we serve.
QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
• Bachelor's degree (BSN) in Nursing preferred.
• Minimum of one 1-year nursing experience; hospital experience preferred.
• Experience in home visits preferred.
• Prefer applicants with 3-5 years public health nurse experience, doing home visits, managing patient case load of up to 50-100.
• Must have minimum 1-year management and supervisory experience, preferably of other public health case managers.
• Experience with grants.
CERTIFICATES, LICENSES AND REGULATIONS
• Active, unrestricted California RN license preferred.
• California Public Health Nurse Certification preferred.
• A valid California driver's license and a good driving record will be required at the time of appointment and must be maintained throughout employment.
• Current CPR certification or attain within 6 months of hire.
• Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test (to include marijuana). Health must be adequate to perform all duties of the position.
• Applicants must be fully vaccinated against COVID-19 and provide proof of vaccination.
• Must pass background check.
KNOWLEDGE, SKILLS AND ABILITIES
• Knowledge of Public Health Nursing intervention and concepts designed to prevent disease as required by local health regulation.
• Knowledge of diabetes.
• Skilled in case management, preferably of diabetic patients.
• Skilled in effective management of subordinates.
• Skilled in working effectively with other team members to coordinate care of patients.
• Knowledge of managing grant objectives, writing reports, conducting meetings to work toward grant objectives.
TECHNOLOGY SKILLS
• Productivity Software: Microsoft Office 365; Word, Excel, PowerPoint, Email & Calendar Management (outlook), Teams (video conferencing and meetings), and Share Point.
• Electronic health records software (NextGen preferred).
• i2i data software preferred.
• Eagle Dream software preferred.
• Utilization of Paycom, Relias Learning, and PolicyTech systems.
