The Health Science Center (HSC) is looking for a Community Health Worker to serve as a member of the Coastal Bend Health Education Center team. Job Summary: The Community Health Worker I, under general supervision, works in the community with specific target populations, working closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

Required Education and Experience:

    • Associates or equivalent combination of education and experience.

    • Two years of related experience in public health, health outreach, and education.

Required Knowledge, Skills, and Abilities:

    • Knowledge of word processing and spreadsheet applications.

    • Ability to multitask and work cooperatively with others.

    • Ability to perform all tasks and job responsibilities safely without injury to self and others in compliance with A&M System and HSC guidelines.

    • Excellent written communication, analytical, interpersonal, and organizational skills.

Required Licenses and Certifications:

    • Department of State Health Services (DSHS) certification, certified Community Health Worker (CHW), or able to successfully complete the CHW certification within three months of employment.

    • American Association of Diabetes Educators (AADE) Diabetes Paraprofessional Level 1 certification, or able to successfully complete the Diabetes Paraprofessional Level 1 certification within three months of employment.

Preferred Education and Experience:

    • Associate's degree

    • Experience working with diverse individuals and communities

    • Four (4) years medical office or health related field. Experience working with diverse individuals and communities.

Preferred Licenses and Certifications:

    • Certified Medical Assistant

Preferred Special Knowledge, Skills, and Abilities:

    • Ability to speak English and Spanish. Security-Sensitive position and finalist subject to a criminal history background check.

    • Knowledge of Health Insurance Portability and Accountability Act (HIPAA).

Other Requirements or Other Factors

    • Verifiable good driving record and reliable transportation.

    • Ability to travel as needed.

    • Ability to lift up to 20 lbs.

    • Strong ability to work with diverse low-income population.

Job Responsibilities

    • One-on-one education of targeted population.

    • Assists patients in reviewing care plans and results in an effective manner while strictly adhering to the policies and procedures in place; motivates patients to be active and engaged participants in their health; builds and maintains positive working relationships with the patients, healthcare providers and community organizations.

    • Confers as needed and on a timely basis with HST staff regarding patient care plans.

    • Evening and weekend hours are required.

    • Travels to various locations for program activities in a personal or University vehicle.

    • Provides culturally appropriate support and information for the patient and their families in a series of structured visits and follow-up phone calls in a variety of settings, including the patient's home.

    • Assists patients in overcoming barriers to obtaining needed medical care and social services; facilitates and coordinates services between health care providers; helps patients in utilizing resources, including scheduling appointments and assisting with completion of applications for programs for which they may be eligible; follows up on pending applications; encourages patients to set goals, identify barriers and challenges and monitor their own conditions.

    • Accurately maintains daily activity logs regarding client activity and destinations.

    • Facilitates communication and coordinates services between providers.

    • Telephones pre and post appointment reminders; telephones recruitment after outreach/educational events; follows up with health management/care plans with both patients and providers.

    • Continuously expands knowledge and understanding of community resources, services and programs provided. Identifies and applies appropriate role definition and skilled boundaries.

    • Other duties as assigned.

    • Work beyond normal offices hours and/or work on weekends.
