As a Registered Nurse Transitional Care Manager with us, you will have the opportunity to be involved in the coordination of care and services for patients admitted to Saint Joseph Hospital / SCL Health who are identified as needing assistance or meeting Care Management criteria.

Your day.

   

As a Registered Nurse Transitional Care Manager you need to know how to:

   
     
  • Work with physicians, surgeons and interdisciplinary teams to facilitate coordinated care, guide the patient and manage program expectations, throughout the pre-hospitalization period to 90 days post hospital discharge.
  •  
  • Transition of care activities include patient assessment and education, risk stratification, discharge planning, development of a post-acute care plan, coordination of post-acute services and direct follow-up on patients progress relative to their post-acute plan.
  •  
  • Performs follow-up tasks identified as part of an episodic payment model or value-based purchasing program for care management of patients across the health-care continuum.
  •  
  • Develops transitional care plan for at least 90 days post hospitalization in collaboration with patient/family and care team which may include but not limited to, physician/surgeon/specialist, PA/NP, hospitalist, case managers and care coordinators.
  •  
  • Develops and maintains pro-active, collaborative partnerships with the post-acute care preferred provider network to manage patients length of stay and optimize recovery in the most appropriate level of care.

Your experience.

  • Bachelors Degree in Nursing (BSN)
  •  
  • Current Colorado RN license or compact license with the ability to obtain Colorado RN license
  •  
  • Four (4) or more years case management/care coordination experience, in either an acute care setting, payor UM department, ambulatory care setting or post-acute care setting. 
  •  
  • Demonstrated experience in data collection, tracking, and reporting 
  •  
  • EPIC experience required
  •  
   

Preferred:

   
     
  • At least five (5) years of nursing experience
  •  
  • Prior knowledge of managed care / utilization review
  •  
  • Master Level Degree
  •  
  • ACSM or CCMC Certification
  •
