Qualifications:

Degree: Masters as a Nurse Practitioner

Certification: Certified as a Family Nurse Practitioner

License: Any US State License

Experience: At least three years’ experience in the outpatient clinic setting as an Advanced Practice Nurse Practitioner. Experience in providing ongoing evaluation of care management services in the primary outpatient setting, Familiar with the Patient Center Model Home (PCMH), secure messaging, AHLTA, CHCS, DMHRIs.
