Your day.




As a Palliative Care Social Worker you need to know how to:




        Assess the patients status and needs, complete appropriate diagnostic techniques/tests to ascertain patients on-going condition, and perform necessary interventions.


        Evaluate the patient in various care setting environments to determine social/emotional status, short and long range planning needs, financial status, ability to function safely within the environment and the ability of the environment to meet the care needs of the patient.


        Utilize various theoretical frameworks to define and interpret patient/family psychosocial issues to multidisciplinary team members.


        Keep an accurate record of patients status and care dispensed and in accordance with the department guidelines and requirements.


        Adjust care given based on changing volume, acuity, cases, census, and department needs.


        Demonstrate patience and care at all times when dealing directly with patients and families.




Your experience.




We hire people, not resumes. But we also expect excellence, which is why we require:




        Graduate of an accredited school of social work, sociology, or related field, required


        Licensed Clinical Social Worker (LCSW), required


        Knowledge and experience with managed care, required


        Minimum of one (1) year of case management experience, required


        Prior Home Palliative, Home Health, or Home Hospice experience, preferred
