Your day.




As an RN Discharge Navigator you need to know how to:




        Coordinate and direct patient care between units/departments.


        Coordinate efficient and effective care between disciplines.


        Stage patients for discharge through the Care Team.


        Follow-up on patient and family questions and concerns regarding their medical condition post-discharge.


        Actively engage and drive rounds on the units.


        Identify resources and roadblocks to insure more efficient patient flow and timely care and discharge.


        Question and prevent duplication, fragmentation, or use of unnecessary resources.




Your experience.




We hire people, not resumes. But we also expect excellence, which is why we require:




        ASN from an accredited program with the ability to obtain your BSN from an accredited program within 4 years of your hire, required


        Current license as a Registered Nurse in the state of practice, required


        Current Basic Life Support (BLS) course completion card (ACLS/NRP/PALS as appropriate for the unit of service), required


        Minimum of three (3) years of nursing experience, required


        Masters of Nursing Degree (MSN), preferred


        Previous nursing leadership experience in a Charge Nurse or Relief Charge Nurse role, preferred
