Nurse Practitioner

The Elmwood Health Center is currently recruiting for both a Full Time as well as Part Time Family Nurse Practitioner to join its provider team in the delivery of high quality patient care to both the adult as well as pediatric patients at this NCQA Certified PCMH Level 3 community health center.

•Performs physical assessments on patients and records findings systematically.
•Orders, interprets and evaluates diagnostic tests to assess and identify patient’s clinical problems and health needs.
•Uses diagnostic screening measures to assist with physical assessment. Interprets findings, and communicates results and management with patients when appropriate and/or care providers.
•Formulates plan and prognosis, based on patient's condition.
•Prepares comprehensive patient care plan based upon findings.
•Coordinates health services and manages on-going health care for assigned patients.
•Documents all pertinent information in Electronic Health Record (EHR).
•Provides health teaching for prevention and/or management of acute and chronic medical conditions.
•Assists in maintaining safety and infection control standards.
•Makes referrals for specialty services as indicated.
•Participates in in-service training of clinical staff.
•Serves as resource to staff on health related matters.
•Maintains professional licensure and certifications.
•Achieves and maintains minimum productivity expectations including, but not limited to, patient billing and contemporaneous charting.
•Provides clinical direction to the nursing staff assigned to provide support to provider.
•Complies with all agency policies and procedures.
•Other duties as assigned.

•Promotes positive image of the EHC and Agency.
•Initiates and problem solves utilizing available resources within level of responsibility.
•Contributes to a positive work environment by interacting in a courteous, pleasant, and professional manner. Functions as a member of an interdisciplinary team through utilization of communication skills and cooperation.
•Demonstrates flexibility by understanding different views. Adapts approach as situations change and accepts changes in the organization and job description requirements.
•Supports EHC/Agency by accepting additional tasks outside of job description such as special projects, public education.
•Plans time off in advance and manages PTO according to department guidelines.
Minimum Requirements  •Current NYS State licensure as a Nurse Practitioner including prescriptive privileges (DEA registration).
•Current license as a registered nurse in the State of New York.
•Masters Degree in Nursing.
•Clinical experience in field of MR/DD preferred.
