Western Colorado Physicians GroupWestern Colorado Physicians Group, a division of Primary Care Partners, Inc., is seeking a full-time LPN Care Coordinator. 


This position is responsible for the care management of a focused group of high risk patients under physician direction. Through diplomatic, clinically sound and professional communication skills, advocates between the provider and patient to lead the patient toward self-management of their care, coordinate nursing care and assure patients are following their care plan.


The right candidate will be responsible for:

  • Monitoring of a designated group of high risk patients.

  • Develop a rapport with patient and the patients family and/or support system regarding the progress with their medical and health care plan. Methods to be used include home visits, office visits, and regular telephone and email contact.

  • Coordinate and manage, proactively, the patients care transitions with other medical offices (specialists), health care facilities (e.g. patients discharged from the hospital) and all PCP and community services that utilized to help patient, This includes having concentrated knowledge of the appropriate utilization of these services and assuring the patient is completing visits/contact with their primary care physician and outside resources as planned/instructed.

  • Participate in patient office visits with the primary care physician, as deemed necessary.

  • Confer with patients primary care physician when significant changes in patients medical milieu occur.

  • Maintains positive interface and relation with those noted in the bullet point above.

  • Coordinate the preparation and presentation of data. Participate on orientation and education sessions as required and needed to fulfill the goals and objectives of the program.

Experience and Education Requirements:


Must be an LPN who is capable of working nursing positions; caring personality with a positive attitude; calm under pressure and exhibit sound judgment; excellent time management and problem solving abilities; good communication skills; able to work as part of the back office team and alone. Must be accountable, must have knowledge for proper use of medical and lab equipment.  Coordination of care between the patient and other community resources. Documented experience in communication with diverse groups (e.g. physicians, a health plan, community resources, nurses, patients and families). Must have skills in patient motivational interviewing and exhibiting a positive attitude under stressful situations. Must be able to multi-task and be capable of prioritizing tasks. Previous medical office experience preferred.


Have appropriate certification of licensure. Attend Continuing Education programs as requested or as required by the position.
