Description:

The Air Force Civilian Service (AFCS) is looking for a Nurse (Healthcare Integrator) to work at 42d Air Base Wing, Health Care Integration, Maxwell AFB AL. The primary purpose of this position is to serve as the Health Care Integrator (HCI) responsible for planning, design, implementation, and evaluation of the Population Health, Disease Management (DM), Utilization Management (UM), Case Management (CM), and Exceptional Family Member Program (EFMP) programs, based on the identified needs of the managed beneficiary population.  The integration of UM, CM, DM, and Population Health is termed Medical Management (MM).  The HCI will provide supervisory duties and responsibilities as directed by the Executive Leadership. 


 


SALARY RANGE: $61,218 - $79,586


KEY REQUIREMENTS:



  • U.S. CITIZENSHIP REQUIRED


 



  • At a minimum, qualification requirements stipulated by the US Office of Personnel Management (OPM) for the 0610 occupational series must be met.


 



  • At least 10 (ten) years of experience as a Registered Nurse (RN) with at least 24 months of experience in ambulatory care is highly desired.  Master’s Degree in nursing, community health, or other health-related field AND national certification in a related nursing field is highly desired.


 



  • Must maintain a current Register Nurse (RN) license from a state or territory of the United States or District of Columbia.


 



  • Must be able to maintain competency in Basic Life Support (BLS) certification.  Certification in Advanced Cardiac Support (ACLS), Pediatric Advance Life Support (PALS), and /or Neonatal Resuscitation Program (NRP) is highly recommended.  Current and ongoing achievement of required Continuing Nursing Education (CNE) to comply with licensure and Air Force requirements,; completion of security background check and MTF Newcomer’s Orientation and Checklist prior to performance of independent patient care; completion of unit-specific orientation, to include full review of 42 MDG I within six weeks of assignment; attendance at Air Force Healthcare Integrator Course within six months of assignment; and completion of 42 MDG required training on annual basis.


 



  • Position is designated as a testing designated position (TDP) under the Air Force Civilian Drug Testing Program.  Incumbent(s) of this position is subject to the possibility of random drug testing.


 



  • Position may be identified as having “regular contact with children”, requiring a State Criminal History Repository (SCHR) check to be completed on any person selected for this position.


 



  • Position may be designated as mission essential based on the local conditions.  Mission Essential personnel report for duty at normal times when the base is closed or under delayed reporting, and remains on duty during Phased or Early Release.  Supervisors make these designations base on mission requirements.


 



  • May be assigned to MTF disaster team; may be required to participate in applicable team training/education and exercises during normal duty hours.  In event of an actual disaster, may be recalled and required to respond to team assignment.


 


DUTIES



  • The HCI is a Medical Group resource that possesses broad clinical experience, strong leadership skills, and advanced interpersonal communication abilities to cross the continuum.

  • Accesses and analyzes numerous electronic databases and other available information sources in regards to population health, clinical preventive, services, DM, CM, and UM.

  • Develops and implements a DM program using existing or newly approved clinical guidelines

  • Facilitates research, development, and implementation of evidence-based clinical practice guidelines (CPGs) that describe and predict the plan of treatment for a specific patient population.

  • Establishes, oversees, coordinates and supervises the CM program.  Ensures appropriate access to quality care and promotes care that is safe, timely, and cost effective by maximizing the use of available resources for those patients identified with complex health need requiring CM.


 


KNOWLEDGE, SKILLS AND ABILITIES (KSA’s) Note:  Your resume and qualifications will be evaluated on the basis of your level of knowledge, skills, abilities and/or competencies in the following areas:



  • Extensive knowledge with application ability of advanced principles and practices of nursing, population-based management strategies, UM, DM, CM, managed care, ambulatory care, and management of resources, prevention, nursing research, medical services and standards to include: NCQA, AAAHC, TJC, AFMOA, AFIs, and MDGIs.


 



  • Extensive knowledge with application of Air Force Medical Services (AFMS) community/population health programs, prevention concepts, and patient-family health education/counseling principles: ability to assess, plan, develop, implement, coordinate, and evaluate prevention services for assigned beneficiaries.  Knowledge of community and facility resources, and referral system.


 



  • Demonstrates knowledge and ability to apply, facilitate, implement principles of DM, CM, UM, and EFMP.  Screens assigned patient population and identifies those appropriate for programs.  Gains required expertise, coordinates/manages MTF-wide programs, and establishes/updates guidelines and 42 MDG based on current national guidelines and best practices and evaluates/reports program outcomes/effectiveness through established metrics.


 



  • Ability to initiate and perform duties independently, as well as to function daily as member of interdisciplinary team includes GPMs, PCM teams, support staff, additional HCI, and other Medical Management members.


 



  • Demonstrates ability to communicate orally, in writing, and utilizing various designated computer information systems and database programs, with 42 MDG leadership, other MTF, base departments/organizations, Humana, and community resources to include, but not limited to: Microsoft Word, PowerPoint, Excel (i.e. V-Look up, detailed graphs, etc), Composite Health Care System (CHCS), Armed Forces Health Longitudinal Technology Application (AHLTA), Air Force Complete Immunization Tracking Application (AFCITA), Aeromedical Services Information Management Systems (ASIMS), Population Health Portal (CarePoint), and Air Force Biometrical Data Assurance Services (BDQUAS).  Demonstrates ability to perform advanced computer skills necessary to analyze, track, and quantify program effectiveness, and to manage and dissemination of information critical to the success of the managed care platform.

  • Ability to review guidelines, and build a program using the parameters of the guideline.
