JOB DUTIES / RESPONSIBILITIES:
•Will respond to direct consults in the Emergency Room Department from physicians and perform medication reconciliation. Attend Code Blues in the Emergency Room Department.
•Receives and interprets medication orders, monitoring for appropriateness of use, dose, allergies, duplication of treatment and interactions (clinical competency).
•Ensuring medication orders are reviewed prior to nursing administration of medication except in emergency situations or areas where physicians are present.
•Orders are clarified as needed to ensure safe medication practices. Investigates unusual doses to validate correctness. Intervene as appropriate to suggest alternative therapies to improve outcomes and lower cost, or to clarify medication orders.
•Clinical monitoring / interventions done as appropriate. Doses are adjusted based on age, renal function and other clinical parameters as appropriate.
•Utilizes verbal order read back when receiving verbal orders.
•Accurately transcribe medication orders and generate computerized medication administration record to be used by Nursing and Pharmacy, assuring correct dose, duration, administration times and instructions (technical competency).
•Dispense medications accurately and within guidelines of appropriate regulations, policies and procedures. Supervises, oversees, checks Pharmacy Technician's work before medication is dispensed to patient.
•Ensuring correct medication and dose available for administration to nurse and physician.
•Uses appropriate aseptic technique when preparing intravenous medications (ie: follows regulations set forth by American Society of Health Systems Pharmacists (ASHP), Texas State Board of Pharmacy (TSBP), hospital policy, USP 797, etc.)
•Follows special safety procedures for high-risk medications (chemotherapy and neonatal doses).
Consult with and advise medical staff and nursing staff to provide information and/or technical assistance as needed.
•During non-peak times, completes other tasks to assist with overall departmental operations
Qualifications/Duties:
Texas Health Harris Methodist Hospital Fort Worth seeks to hire a Clinical Pharmacist in the ER Pharmacy Department
Full Time: 40 hours per week
7 on/7off 3rd shift.
Minimum Education Requirements:
A minimum of a Bachelor of Science in Pharmacy or Pharm D degree from an accredited College of Pharmacy
Minimum Licensure/Certification Requirements: Registered pharmacist in good standing with the Texas State Board of Pharmacy.
Minimum Experience Requirements:
Three years hospital experience or PGY1 (Post Graduate Year 1 Residency) is strongly preferred.
Minimum Skill Requirements:
Basic data entry skills, excellent English language verbal/written communication skills.
