JOB DUTIES AND RESPONSIBILITIES


As a Nurse Practitioner you will:



 



  • Performs an initial comprehensive assessment on all newly enrolled patients in their home reviewing all their needs 

  • Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions

  • Educates members and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness

  • Establishes and maintains effective relationships with patients and their legal representatives, facility staff, PCPs and other collaborating physicians/providers

  • Manage patient care on a longitudinal basis, with the primary objective of delivering proactive chronic disease care and significantly reducing avoidable hospitalizations

  • Diagnose, prescribe and treat within the scope of the Nurse Practitioner and consistent with standard of care

  • Partner with the patient’s primary care provider and specialists to deliver integrated care

  • Participate in a rotated on-call program for home-based patient support after-hours



QUALIFICATIONS




  • Completion of Nurse Practitioner (NP) accredited program.

  • 2+ years of clinical experience with home based care desired 

  • Knowledge of clinical standards of care

  • Knowledge of CPT, ICD-9 and HCPC codes.

  • Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.

  • Current unrestricted Nurse Practitioner state license, and current state driver’s license

  • Passion for geriatric care and patients

  • Comfort with conducting house calls
