As a Nurse Practitioner on this team, you will help patients with multiple medically complex chronic illnesses. This role requires highly skilled Nurse Practitioners or Physician Assistants who are comfortable making decisions autonomously when necessary. Nurse Practitioners and Physician Assistants on this team are challenged to perform at their best every single day.


In this role, you will be able to take the time you need to understand and treat the root causes contributing to your patients’ ongoing symptoms and episodes. You will be able to identify the factors limiting their ability to recover, and help the patient understand the actions needed to make long-term, life-changing improvements.


Often these patients, among the frailest in our nation, are disheartened about their ability to regain control of their health, and in some cases, may have already given up. You can change their stories.

JOB DUTIES AND RESPONSIBILITIES




  • Performs an initial comprehensive assessment on all newly enrolled patients




  • Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions




  • Educates members and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness




  • Establishes and maintains effective relationships with patients and their legal representatives, facility staff, PCPs and other collaborating physicians/providers



  • Participate in a rotated on-call program for home-based patient support after-hours


QUALIFICATIONS




  • Completion of Nurse Practitioner (NP) accredited program




  • 2+ years of clinical experience




  • Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful.



  • Current unrestricted Nurse Practitioner state license, and current state driver’s license
