Facility Care Manager
(Registered Nurse)
Currently looking for a Facility Care Manager to fill a full time opportunity in Salem. Will be responsible for providing a broad range of care management services for correctional clients to include: utilization of medical services, quality of care, risk management and catastrophic case management.

Essential Functions :

  • Review concurrent and retrospective inpatient and/or subacute admissions for utilization based on Milliman Care Guidelines criteria, quality improvement, risk management, and discharge planning needs.

  • Develop and maintain a good working relationship with the healthcare team, to include correctional institution personnel.

  • Identify, communicate and document system-wide quality and cost containment measures.

  • Assess patient's medical and psychosocial needs to develop, communicate and implement a comprehensive plan of care in collaboration with the healthcare team.

  • Document pre-authorizations, concurrent and retrospective reviews, care management interventions and discharge plan in appropriate care management system.

  • Adhere to CHP Care Management Standard Operating Procedures.


Other Duties / Responsibilities :

  • Participate in Medical Director conference calls.

  • Participate in weekend on-call schedule rotation as needed.

  • Participate in LEAN quality improvement initiatives.

  • Other duties as assigned.


Education :

  • Graduate of accredited school of nursing with BSN (may be waived for individual who has an RN, SW degree or diploma associate degree in nursing and demonstrates experience in quality improvement, utilization management or discharge planning).


Experience :

  • Three (3) to five (5) years clinical nursing experience.


Requirements :

  • Must have a valid driver's license and reliable transportation.

  • Must be willing to travel to correctional institutions state-wide at least monthly (75% travel).


Knowledge :

  • Knowledge of care/case management.

  • Knowledge of community resources/agencies.

  • Knowledge of NCQA, QA, Medicare/Medicaid guidelines.

  • Knowledge of program and workflow development, process improvement and implementation procedures.



  • Skilled in conflict management to assure problem prevention and resolution.

  • Skilled in developing and maintaining positive relationships and communicating effectively with internal staff and external customers.

  • Skilled in organization, coordination and facilitation of meetings and/or events.


Abilities :

  • Able to perform basic to intermediate level of competence with various computer software applications including MS Outlook, Word, Excel, Access and Power Point.

  • Able to effectively communicate with internal staff and external customers.

  • Able to solve resource issues in a creative, positive manner to improve clinical outcomes of patients.

  • Able to work with correctional institution PCP and health care team members to most effectively manage patients.

  • Able to participate in program development and identify appropriate goals of program.


PHYSICAL/MENTAL DEMANDS: Requires sitting and standing associated with a busy office and hospital environment as well as stooping and bending to file and maintain orderliness; mobility to complete errands. Must be able to assess, evaluate, and respond to acute issues in a timely, competent manner. Must be able to function with frequent interruptions, accept change and able to prioritize activities when faced with competing demands.
ENVIRONMENTAL/WORKING CONDITIONS: Normal busy office environment and normal hospital environment.
