Sierra Donor Services (SDS) is looking for a dynamic and enthusiastic team member to join us to save lives!! Our mission at SDS is to save lives through organ and tissue donation, and we want professionals on our team that will embrace this important work!! We want people to join our team in the role of Inhouse Recovery Coordinator with previous experience as a registered nurse (RN) in an ICU or critical care setting. This position will be the onsite Organ Recovery Coordinator at Santa Rosa Memorial to facilitate all aspects of making organ donation happen.

What is an Organ Recovery Coordinator?

Using their nursing background, Organ Recovery Coordinators (ORCs) oversee the process of procuring the gift of organ donation. In collaboration with the clinical team, ORCs assess the compatibility of organs for recipients, and expedite the surgical and technical processes to package and transfer the organs for transplant quickly and safely. They work alongside hospital personnel and surgeons to facilitate the lifesaving gift of organ transplantation.

Key responsibilities this position will perform include:

Evaluates patient suitability for organ donation by reviewing medical records and consultation with medical staff.
Coordinates organ placement and allocation with transplant programs and surgeons.
Coordinates and assists in the surgical recovery of organs and peri-operative management – including logistics such as arranging transportation, ensuring surgical packaging, and preparing lab specimens for shipment.
Provides support to donor families and conducts medial/social history interviews.
Partners with hospital leadership to improve donation metrics, protocols, and policy. Participates in committees to influence donation opportunities.
Develops and presents hospital educational programs and status reports based upon strategic goals.
Updates donor information into electronic medical record.
Other duties as assigned.

The ideal candidate will have:

Two plus years prior experience in a critical care setting.
Ability to exercise independent judgement and multitask.
Exceptional teamwork, interpersonal, communication, and conflict management skills.
Registered Nursing (RN) License

The Medical Management department is participant focused and strives to provide the best possible care for the participants through Utilization Review and Utilization Management services, Care Coordination and Population Health Management. The Medical Management department also assists participants in receiving care with specialists outside the service area when appropriate. The Department is designed to ensure the delivery of high quality, cost efficient healthcare for our participants and families through coordinating care, providing detailed discharge plans, advising participants of different programs available and providing face-to-face and telephonic education.

The purpose of this position is to ensure that the Fund’s philosophy of assisting patients to receive the best quality care in the appropriate setting is accomplished. Will work with participants in the acute hospital and lower level of care settings to help guide them through the continuum of care. Will assess, plan, implement, coordinate, monitor, and evaluate care options and services to meet our participants’ health care needs. In conjunction with the Transition Management Team, this position is responsible for ensuring that the discharge plan is achieved in a timely manner and effectively meets the patients’ needs.

ESSENTIAL JOB FUNCTIONS AND DUTIES

Work with participants of all ages in the acute hospital and lower level of care settings (LTAC, SNF, Rehab) to help them through the continuum of care

In collaboration with the attending physician, responsible for continuous communication with all parties involved in the plan of care and decision-making to determine medical necessity and appropriateness of care for service utilization and expense

The AHP has responsibility for the standards of care for their specified patient population and evidence self-direction and accountability both to quality of care and in the development of their role within all UCSF Health locations and affiliates.

Duties and tasks

The Contractor shall perform a full range of professional nursing services and practices on site, using government furnished facilities, equipment and supplies to a variety of patients. All aspects of Nursing Services shall be commensurate with the capabilities and licensure of the provider and the Medical Department including but not limited to the expectations listed below; and in the other areas of the PWS.

A. Assesses patient's needs for nursing care, plans for nursing interventions, assess the effectiveness of the interventions and alters the nursing plan and interventions appropriately.

B. Provides patient education, disease management assistance and ongoing monitoring as directed by the Primary Care Manager.

C. Provide nursing triage to acute issues in addition to regular scheduled appointments, with immediate evaluation and management of emergent problems as they occur.

D. Administers oxygen, IV fluids, and prescribed oral, subcutaneous and intra-muscular medication in accordance with provider orders and regulations.

E. Prepares patients for surgical and or diagnostic procedures.

F. Prepares, operates, and monitors specialized equipment such as cardiac monitors, defibrillators, etc.

G. Inventories medicine and drug stocks to ensure adequate supplies and controls.

H. Participates in training to clinic staff; Provides instruction on an ad hoc basis.

I. Participates in the Health Services Quality Improvement Program, as directed by the Clinic Administrator.

J. Prepare and submit appropriate documentation as specified in applicable regulations.

K. Provide coordination of care between civilian, DOD and CG facilities in accordance with standards of practice established at the clinic for referral management.

L. Communicates and coordinates with HSWL-SC Operational Medicine Nurse Consultant regarding complex care oversight.

Experience: A minimum of two (2) years within the past five (5) years in the practice of clinical psychology , in a U.S. Government setting such as a DOD or Department of Veterans Affairs (VA) MTF and providing clinical services to individuals, couples or groups in clinical psychotherapy services. The Contractor shall ensure that the contract service provider has sufficient training and experience to effectively provide Specialty services for service members and family members. Related experience will not be accepted in lieu of degree or license requirements.

7.4. Basic Life Support Training: All contract service providers shall possess and maintain current course completion documentation for basic life support (BLS) through an American Heart Association and/or Red Cross approved training program. BLS shall be renewed every two years.
