Hours are Monday-Friday 7:30AM-4:30PM

| Education / License / Certifications: |
Have graduated from a physical therapy assistant program accredited by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association (APTA)
US CITIZENSHIP
Licensure in any of the 50 states
